Patient Information

Recall Visits

Regular preventative care is the key to maintaining good dental health. It is your responsibility to follow prescribed daily care and return for scheduled treatment. We will schedule your recall appointment to ensure you maintain the best oral health. At the time of your recall visit, please advise the office staff of any change in your:

  • Address or telephone number,
  • Marital status,
  • Employment and insurance coverage.

Dental Insurance, Fees and Payments

Dental insurance is intended to cover some, but not all, of the cost of your dental care. Most plans include coinsurance, a deductible and other expenses which must be paid by the patient. If you have dental insurance, please bring your plan information with you to ensure that you receive the maximum benefits to which you are entitled.

We accept many dental insurance plans and are a Delta Dental Provider.

If you have any questions regarding your coverage, please contact us here or ask during your visit.

We make every effort to keep down the cost of your dental care. You can help by paying for treatment at the time of your visit. If your treatment program requires several visits, you will be given an estimate of the total treatment. Our office does not want you to be surprised by any unexpected costs.

Payment Options

We also accept Mastercard, Visa, Discover and Care Credit for your convenience.
